Please have the following information ready in order to complete this form:
- your credit card (after you click submit you will be taken to a payment screen)
- names of all booth attendees
- a company logo (180px by 180px)
- a banner image of 600px by 110px (optional for $50, but encouraged)
- the number of banquet tickets you’d like to purchase for Friday night ($25 per attendee)
Also needed, but not necessarily today: a pdf certificate of liability insurance showing liability limits of at least $1,000,000 per occurrence (needs to show the Iowa Bandmasters Association as an ‘Additional Insured’ on your policy). This can be uploaded to the online form when you register, or it can be emailed directly to exhibitschair@bandmasters.org at a later date.
